This is a charity basketball 3-on-3 tournament hosted by 211 Premium Sports and presented by Spear Physical Therapy at the Dwarf Giraffe Gymnasium in Whitestone, NY, with the intention towards veteran suicide prevention. All proceeds from the sale of food, beverage, and raffle tickets will be donated to Guardian Revival, a registered 501 (c)(3) chartered to revive and preserve the mental health and well-being of veterans and first responders — our guardians — at no cost to them. We encourage you to click the link above and visit there site to learn more about their work and how you can contribute to the prevention of veterans from taking their own lives.
The tournament will be a round robin to start with the top 4 teams making it to the "final bracket". Games will be 3v3 Half Court and will be officiated. Games will be decided by the first team to score 16 points (scored Via 1s and 2s), OR 15 Minute running clock (whichever is achieved first) Please RSVP with all team members and a team name by December 12th.
Teams will be allowed to have 3 players on the court at all times and are allowed to have up to three additional reserves. Teams are free to make substitutions during any dead-ball situation (after a call is made or a point is scored).
Entrance Fees for each team are $310. Must be 18 or older to enter. Payment may be made via venmo (at the end of this form) or via cash day of. All payments made via venmo will secure spot in tournament, all teams option to pay cash are subject to having their spot filled. All Players are required to sign a liability waiver, which will be emailed to the captains, along with a full "Rules of Play". Please bring all waivers to the gym on day of the tournament.
